All applications for search warrants must include the following—

(1) the identity of the person whose property, premises, messages, or channel is to be searched, or is otherwise the target of the search warrant;

(2) the location to be searched;

(3) a sworn affidavit, made under penalty of perjury, outlining why the applicant believes there is probable cause; and,

(4) the exact violation of law being alleged and a description of the alleged conduct.
