Any warrants issued for the arrest of any person shall list—

(1) the name of the subject of the warrant;

(2) the criminal offense or offenses for which the subject is being arrested, including any applicable citations of this Code, or of other laws;

(3) the name of the court which issued the warrant;

(4) the name and signature of the Justice or judge who signed the warrant;

(5) the case number or citation regarding which, or for which, the warrant was issued; and,

(6) the date on which the warrant was issued.
